Promotion Rules and Limitations
Offer begins November 1, 2009 and ends December 31, 2009 12 p.m. PDT.
• Qualifying lens pairs must be ordered on Eyefinity’s website (Eyefinity.com) during the promotion period, and dated November 1 through December 31, 2009.
• All complete pairs of KODAK Unique Lenses in any lens material are entries into each weekly drawing. Four drawings per week for a total of 36 drawings and prizes awarded.
• Weekly drawings will include orders from Sunday-Saturday of previous week during promotion period.
• Prize includes: One Fifty Dollar Value ($50) American Express Gift Card.
• Limit one prize per Eyefinity account number.
• Prizes will be personally delivered by a local Signet Armorlite sales rep if available or shipped directly to the Eyefinity account name and address on file.
• Void where prohibited by law. Please check legality in your state before participating.
• All lens sales that are paid in total or in part by a federal health care program (e.g. Medicare or Medicaid) are excluded from qualifying for a prize.

Qualifications
To qualify for PracticePlus enrollment, you must be an eye care professional who is qualified to dispense prescription eyewear to consumers. Only Kodak Progressive Lenses qualify and they must be purchased through a SA authorized independent wholesale lab in either the US or Canada. Signet Armorlite, Inc. reserves the right to deny any enrollment not meeting qualifications or to discontinue PracticePlus at anytime.
